Activity Insert Perpetual Plaque features 12 individual engraving plates and a choice of activity insert.

Custom insert option available.

The individual Mini-plates measure 2.75" x 0.9" and are attached to the plaque with small gold-plated screws.

This plaque is great for lots of activities. Doesn't matter it is in a school, a formal corporate setting, or for a serious sport or other entertainment, this simple yet universal plaque will meet your needs!

Engraving text on header plate included in these prices.

Engraving on mini-plates is $3.00 per plate for two lines or $4.50 per plate for three lines unless pre-paid at time of plaque purchase.

Want us to ship blank plates?

Choose the "Hold until I need" option and we'll hold your unengraved mini-plates in stock until you need them. Otherwise you'll have to mail blank plates back to us for engraving.

Lost or damaged plates cost $5.00 each.

Does your Engraving Text have a Logo?:

We will add a one-time charge of $10 per logo in your design. If we already have your logo on file you will not be charged.
